Flag Fin Angle sudden death???

Hi,
My flag fin angle died yesterday very unexpected,he ate last night and was no change, I have no idea why I have had him for about 6 months plus has eaten since the day we got him home, misies, flake, pellet, sheet algae,. Water has been perfect for months amo0, niti0, nitra, below 2, alk, 3, sal 1.023, no changes just the addition of a couple uv led in my fixture plus a couple red and green, I have not even done my water change yet this week, I use rodi and Red Sea coral pro with 5% weekly water change. I have no ideas the tank is a 92 corner ? Any thoughts thank you
 
Stray voltage?
Contaminants leaking (cracked heater)?
Do you have any other live stock/corals? If so how are they doing?
 
I have my entire set up on a dedicated gfi circuit my corals have been closed for awhile an have not opened really all other fish seem to be doing fine ? I run a reactor witha carbon gfo mix 1 table spoon gfo the rest carbon. I have no idea I also have a ato tapped off my rodi filter to come on once a day ? I will volt meter the water to see if I have some stray voltage but I'm sure the gfi would have tripped
 
Sometimes a fish will just die like that... Out of nowhere. Most of the time, you will never know why.
As for the stray voltages, you can measure it if you have a meter to do so. Sometimes there is just a tiny amount of stray voltages in there, so it won't shock you when you stick your hand in the tank. But if you have a cut in your finger, stick that finger into the tank. Even a small amount of current in there, you will feel it in your cut right away.. Its like tiny needles stick into your flesh.
 
When did you find the fish had died?
Is there the possibility you could have lost power for a while?
 
I measured the water to ground on voltage and had a 0 volt reading I know a gfci is designed to trip at .005 amps so any leakage I would expect to pick it up on the gfi. My tank lately seems super clean like 0 algae any were is it possible there is not enough nutrients in the water colum should I skip a week with water change ? The only algae in the tank is some slim in the refugium on the cheato. I have a kenya tree that has never opened once so I think something could be up I just don't know what I have even had my LFS test my water against my own readings they were all dead on. I have no idea the tank looks perfect otherwise
 
No power lose I have a apex controller temp has been stabile with in .5 degree and no power outage recorded. All other live stock is great I have a dwarf lion could he maybe have stung him at night ?
 
To much light I have a custom led I bought with the tank it has over 50 led on it not sure on the wattage they look like your standard 3w led

Hmmmm, sounds possible. Especially where everything else seems up to par.
Or like Bao mentioned, maybe it was just his time :(
