My new orange-lined cardinalfish died. =\ Question on possible causes?

So I bought an orange-lined cardinal fish last Saturday that seemed to be healthy and happy and I slowly acclimated him to my tank like all my fish get treated to. Well Saturday and Sunday he was shy and hid in the back behind rocks as most new fish do. However by late Sunday he came to the front of my tank and by Monday and Tuesday he was out and about all over the entire tank. He had been eating nice and healthy on Saturday (small amount to see if he'd accept my brand of food) then normal eating on Sunday and Tuesday. Showed no signs of stress, and was never seen bullied by any tankmates what so ever.

Found him in the back of the tank in the rockwork Tuesday night being devoured by my cleanup crew. No clue what caused the death. I did notice Tuesday morning that he had some pale areas on him and was worried about it but didn't think it'd cause death.

Now I have had many multiple fish added into my tank since April. Each and every fish survived and is still alive today, except, oddly enough, a pajama cardinalfish, an orangelined cardinal, and a fire goby. So 2 cardinalfish species and a flame goby. All of which are known to be cave dwellers and hide in rockwork. Maybe it's a coincidence, but does anyone have any ideas what the connection, if any, could be causing this? I'd go out and buy another OLC this weekend but I don't want another one passing away due to something I could've avoided. I have very peaceful tankmates and even my little clown goby, tiny lil guy, never gets bothered.
