Run a UV sterilizer appropriately sized for your system and the manaufacturers recommendation. I use a 40 watt Pentair and have never QT'd a fish in my life
Buying already QT'd fish is a good option as well. They're usually a bit pricier.
These were not sure what happen
If you added anything not qtd afterwards then that will do it , also tank needs to sit fallow for 76 days or do all qtd fishes from the beginning
