Another bad Coral Banded Shrimp Molt!

It appears that I've lost Jacques, a coral band shrimp to a molt. This is the second lost CB shrimp in 6 months. :(

Jacques was a large CBS, and was with me for over a year, and has had other sucessful molts.

Prior to this, I had the same thing happen in my other tank with a smaller CBS.

What appears to happen is that the CBS molts, but seems like it leaves it's claws in the molt. While I'm not SURE this one is gone, it doesn't look good when you see 2 CBS claws under a rock, next to a molt, and no shrimp.

Anyone with any thoughts on why the molts are going bad?
 
Do you use any kind of supplements in your tank? I think certain trace elements are necessary for proper molting. Back in my early reefkeeping days, I would rely soley on water changes, and any kind of shrimp or crab would only seem to last 6 months to a year in my tank. Eventually I started using B-ionic and there was a noitceable difference in the long-term health health of my shrimp, crabs, snails...
 
supplements

whenever i have shrimp, i add iodine once in a while as i was told this helps with molting
 
B-ionic is a primary supplement......but I do not test for or supplement iodine.
