PFO CF Ballast Replacement

Last night, one of my dual ballast didn't fight. after switching cords around, I realized one of them was dead. where can I buy the replacement?

Just a question.. but does the ballast have a fuse? Possibly replace that?

Thank you. I just contacted with PFO. It will cost $50 for a replacement + shipping. Fortunately one of my CF ballast is still working. I posted here to hear people's opinions and options before I order one from PFO.
